This page is designed to help you with your Annual Pastors Report and to assist you in accessing the forms you need to recommend students for District License, Transfer members to another Church and to access District Ministers License and Ordination application.

Our Church year ends the last day of February, so I would recommend that you have your annual meeting, to elect officers and delegates to the various conventions and the District Assembly, some time before March 15th.

Our reporting will be done electronically in order to save time and trees. Please download the appropriate forms by clicking on the Document Downloads link from the Main Menu. It would be a good idea to download these forms to a file on your hard drive so you will have access even when you are not online. You will need to mail me your certificate of election, recommendation for District Ministers License, Ordination application, and any others that need you or other signatures, the rest I would like you to email to me at This e-mail address is being protected from spambots. Please help me by completing your online Annual Pastors Report by April 1st.
I will also need you to fill out and email me the following forms, Church Directory, and Deceased Members List by April 15th. Please do not snail mail me these two forms because if you do I then have to input all the data manually.

Please Mail me your Certificate of Election form filled out and appropriately signed by April 10th.

You will also need to look at the New Church Sponsorship Criteria, General Superintendents Growth Award Criteria to see if you qualify. If you do, I need to be informed as soon as possible as there are process I need to engage to make sure these are recognized at the Assembly.
